Vaccine Mandate Survey

Welcome to Children’s Health Defense’s (CHD’s) survey about experiences our subscribers have had with vaccine mandates. This survey is part of a research project titled “Understanding the Medical Freedom Movement: Experiences and Consequences of Covid Vaccine Mandates.” The lead researcher is Evelyn Bush, a CHD Project Fellow and Associate Professor of Sociology at Fordham University.

As with any research study, before you decide to participate, you have the right to be informed of risks and of your rights as a research participant.
